Medicare Advantage plans in Alabama for 2027

In Alabama the smallest out-of-pocket cap belongs to the plan with the steepest premium, so the cheapest plan on paper is rarely the cheapest year.

Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read

Alabama residents with Parts A and B can choose among 65 Medicare Advantage plans open to the general public for 2027, filed by 7 parent companies, and 36 of the plans that include drug coverage charge nothing per month. The in-network out-of-pocket maximum runs from $1,900 to $9,850, with a median of $6,500 against $6,550 nationally. The plan with the $1,900 ceiling, Humana Gold Plus H4461-073 (HMO), costs $146 a month, the highest premium in the state. 34 plans earn four stars or better for 2027 and the top score is 4.5. Rural counties fare better here than in many Southern states: the thinnest county, Tuscaloosa, still has 21 open plans, while Mobile leads with 46. There are also 48 special needs plans and 11 stand-alone Part D plans, all listed below.

Premium against ceiling: the Alabama trade-off

Look at the two ends of the Alabama list and the same pattern keeps showing up. Humana Gold Plus H4461-073 (HMO), from Humana, promises that in-network spending on covered medical care stops at $1,900 for the year, but it bills $146 every month to get there, which comes to $1,752 before any copay. At the other end, HumanaChoice H5216-269 (PPO) charges $0 with a 4-star rating and a $4,450 cap. A healthy year favors the second; a year with a surgery and a hospital stay can favor the first. The MOOP, short for maximum out of pocket, is the figure that decides the bad year, while the premium decides every month.

Which companies file the most plans

Humana Inc. (15), Devoted Health, Inc. (13), Health Care Service Corporation (12), CVS Health Corporation (8), UnitedHealth Group, Inc. (8), Triton Health Systems, L.L.C. (6) account for the largest counts of open plan segments. Triton Health Systems is the one regional parent among the national groups on that list. The plan that scores highest on the 2027 ratings, Humana Gold Plus H4461-078 (HMO), holds 4.5 stars at $0 with a $6,360 cap; the word giveback in a plan name means it pays back part of your Part B premium.

From Mobile Bay to the Black Belt

Mobile County lists 46 open plans and Jefferson County, around Birmingham, 43. In the Black Belt and the southwestern counties, the menu shrinks to about half that size, with Tuscaloosa at 21, yet no Alabama county is left without a choice of HMO and PPO. A PPO lets you see out-of-network doctors at a higher copay; an HMO generally does not, outside emergencies. The state splits almost evenly: 33 HMOs and 32 PPOs.

Plans for people on Medicaid or with chronic illness

Of the 48 special needs plans, 28 are D-SNPs for people who qualify for both Medicare and Alabama Medicaid, 17 are C-SNPs for conditions such as diabetes or heart failure, and 3 serve nursing-home residents. Compared with 2026, the open list moved from 63 to 65 plans (2 more plans). See how special needs plans work before you switch.

Questions people ask

Why does the Alabama plan with the lowest out-of-pocket maximum cost so much each month?

Humana Gold Plus H4461-073 (HMO) caps in-network costs at $1,900 for 2027 but charges $146 a month, the top premium among Alabama's open plans in the CMS landscape file. A low ceiling shifts cost from copays to premiums, so it pays off only if you expect heavy use of care.

Do rural Alabama counties have fewer Medicare Advantage choices for 2027?

Yes, but the gap is moderate. Mobile County has 46 open plans and Tuscaloosa County, the thinnest, has 21, according to CMS data of September 2026. Every county keeps at least one HMO and one PPO. Open the county in the table to see each plan with its premium and cap.

How many stand-alone Part D plans can Alabama residents buy for 2027?

11 stand-alone drug plans are offered statewide, with premiums from $5.30 to $204.60 a month; the cheapest is Wellcare Value Script (PDP). 2 of them cost nothing for people with full Extra Help. They suit people who keep Original Medicare, with or without a Medigap policy.
