The lowest in-network out-of-pocket cap filed anywhere in the state is $5,900, roughly the national median of $6,550, and the median South Carolina cap is $9,250. Premiums are the strong side: 37 drug-covering plans charge $0. The Upstate leads on choice, with Greenville (51) and Spartanburg (51); the Pee Dee trails, with Darlington at 28. Beaufort County, around Hilton Head, has 32. 35 plans rate four stars or more, and the top is 5. Nationally the lowest cap filed for 2027 is $199. South Carolina's file has nothing close: BlueCross Blue Basic (PPO), from Blue Cross Blue Shield of South Carolina, and DEVOTED CORE 001 SC (HMO) share the lowest ceiling, $5,900. Half the open plans sit at $9,250 or more, close to the $9,850 federal limit. For a healthy year this hardly matters. For a year with a heart procedure or cancer treatment, the cap is the bill.

Five stars, high cap
Clover Health LiveHealthy (PPO) is the state's top-rated open plan at 5 stars, with a $0 premium and a part of the Part B premium given back each month. Its cap is $9,850. Ratings describe quality and service; they say nothing about the worst-case cost, so read both columns in the county table.
Upstate, Midlands, Lowcountry
Greenville 51, Spartanburg 51, Pickens 49, York 48, Anderson 47: the I-85 corridor carries the most plans, with Berkeley and Dorchester, outside Charleston, close behind at 47 each. Charleston itself has 46, Horry, around Myrtle Beach, 41. Darlington (28), Chesterfield (30), Beaufort (32) have the fewest. The parent companies with the most filings are Humana Inc. (20), CVS Health Corporation (11), Health Care Service Corporation (10), BlueCross BlueShield of South Carolina (BCBSSC) (7). The state had 69 open plans in 2026. PPOs outnumber HMOs here, 44 to 23, which suits people who see doctors in two states.
