Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read

Texas has 228 Medicare Advantage plans open to anyone with Parts A and B for 2027, 44 fewer than in 2026, and 193 special needs plans next to them, 119 of those for people who also have Medicaid. In Harris County, the special needs plans (50) now outnumber the open ones (49). The big metros still lead on choice: Harris (49), Bexar (48), Montgomery (43), with Dallas at 42. 21 rural counties have only 4. 166 drug-covering plans charge no premium, and the cap on in-network costs reaches as low as $1,500, on MedCare Classic (HMO) in the Houston area; the median is $6,500. 138 plans rate four stars or more, the top at 5. Humana Inc. and UnitedHealth Group, Inc. file 56 and 50 plans. 10 stand-alone drug plans follow the county table.

Special needs plans, the other half of the Texas market

A D-SNP, or dual-eligible special needs plan, is a Medicare Advantage plan limited to people who have both Medicare and Medicaid. Texas has 119 of them, more than many states have plans of any kind, plus 67 chronic-condition plans. Along the border they matter most: El Paso has 28 D-SNPs against 42 open plans, Hidalgo 20 against 35. People in these plans usually qualify for Extra Help with drug costs too, which turns Part D copays into small fixed amounts.

Where the 44 exits landed

The loss was concentrated in the metros. Harris County saw 23 plan IDs from 2026 not filed again, Collin 25, Dallas 20, Bexar 17. Most people in those counties still have dozens of choices, but a member of an ended plan has to act: the insurer's notice opens a special enrollment period (42 CFR 422.62), and the county pages list every ID that ended.

Low caps around Houston and Dallas-Fort Worth

MedCare Classic (HMO), from MedCare Partners Health Plan of Texas, caps in-network costs at $1,500 with a $0 premium. Alignment Health ONE 011 (HMO-POS) offers $2,800 with 4.5 stars. In Dallas, Tarrant and Collin, the lowest cap is $2,399. Physician-group HMOs drive these numbers, so the network is narrow.

West Texas and the Panhandle

Brewster, Childress, Collingsworth, Cottle, Culberson, Donley have 4 open plans each, mostly PPOs from national carriers. Humana Inc. (56), UnitedHealth Group, Inc. (50), CVS Health Corporation (31), Devoted Health, Inc. (24), Health Care Service Corporation (20), Baylor Scott & White Health (11) file the most plans statewide; Baylor Scott & White is a Texas health system with its own plans.
