Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read

Tennessee bucks the 2027 trend: while the national count of open plans fell from 3,790 to 3,589, the state gained, from 93 open-to-all plans in 2026 to 105. The other standout is the floor on out-of-pocket costs. Humana Gold Plus H4461-081 (HMO) caps in-network spending at $1,900 for $90 a month, and Humana Gold Plus H4461-080 (HMO) offers $3,800 at $0 with 4.5 stars. The statewide median cap is $6,200, under the national $6,550. 66 drug-covering plans charge no premium. Shelby County (Memphis) has the most open plans, 42; Davidson (Nashville) has 37 and Knox (Knoxville) 38, while Bedford County has 19. Devoted Health, Inc. files 25 plans, ahead of BlueCross BlueShield of Tennessee and Humana with 17 each. 50 special needs plans, 27 of them for chronic conditions, and 11 drug plans are listed below.

East Tennessee's low ceilings

Knox County received 11 new plan IDs for 2027, more than any of the big Tennessee counties, and its lowest cap is $1,900. Chattanooga's Hamilton County reaches $2,900. In Memphis and Nashville the lowest cap is $3,400. A low cap is most valuable to someone who expects a surgery or a hospital admission, and that is where Tennessee plans differ most from Original Medicare.

What Original Medicare charges for a stay

Without a plan or supplement, a 2026 admission costs the Part A deductible of $1,736, then daily coinsurance once the stay passes 60 days. Medicare Advantage plans replace that with their own daily copays, which count toward the cap. Compare a plan's hospital copay in its Summary of Benefits with the figure below.

Growth in the suburbs, cuts in the middle

The gain is not everywhere. Davidson County went from 44 to 37, and Bedford County, south of Nashville, from 25 to 19. The counties with the longest lists are Shelby (42), Fayette (40), Williamson (40), Wilson (40). 57 HMOs and 48 PPOs make up the field.

Who files the plans

Devoted Health, Inc. (25), BlueCross BlueShield of Tennessee (17), Humana Inc. (17), Health Care Service Corporation (14), CVS Health Corporation (12), UnitedHealth Group, Inc. (11): 8 parent companies in all. The state's Blue Cross plan competes county by county with national carriers, and a newer entrant now files the most plans. A long filing list does not mean a large membership; it often means one company split a county into several premium and network variants.

Chronic-condition plans

Tennessee has 27 C-SNPs, special needs plans limited to people with conditions such as diabetes or chronic heart failure. They tailor drug lists and care coordination to that condition. You need a qualifying diagnosis to join, and they are listed apart from the open plans in each county.
