Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read

Delaware offers 24 Medicare Advantage plans open to anyone with Parts A and B for 2027, the same count as in 2026, and 11 of the plans with drug coverage carry no monthly premium. New Castle County, around Wilmington, has 23 of them; Sussex, on the coast, 17; and Kent, around Dover, 16. No plan in the state sets its in-network out-of-pocket maximum below $5,900, and the median is $7,150, well above the national $6,550. PPOs make up 16 of the list and HMOs 8. 10 plans hold four stars or more for 2027, topping out at 4.5. Delaware also lists 14 special needs plans. Beneficiaries who keep Original Medicare can choose among 9 stand-alone drug plans, from $5.30 a month, with a median of $36.60.

One floor for the whole state

Every Delaware county shows the same lowest cap: $5,900 in New Castle, $5,900 in Kent and $5,900 in Sussex. That floor belongs to HumanaChoice H5216-390 (PPO), at $7.90 a month. In practice it means the worst year under any Delaware plan costs several thousand dollars, so the cap matters less for choosing than in states where it ranges from a few hundred dollars upward. What differs is premium, network and rating. The tool below shows what a cap of that size is still worth against Original Medicare without a supplement.

A top-rated plan with the highest cap

Humana Essentials Plus Giveback (PPO) reaches 4.5 stars for 2027 with a $0 premium, yet its cap is $9,850. Star ratings measure quality of care and service, not what you pay, so a high score and a high ceiling can coexist. If your health is uncertain, compare the cap first and the stars second.

Wilmington, Dover and the beaches

New Castle has 11 $0 plans with drugs, Sussex 6 and Kent 5. For people who retire to the Sussex coast from another state, a plan bought in the old state ends when you move, and the move opens a special enrollment period to choose a Delaware plan. Doctors across the state line in Salisbury or Philadelphia may be in network for some PPOs, which is worth checking in each directory.

Companies on the list

UnitedHealth Group, Inc. (5), Humana Inc. (5), Highmark Health (4), CVS Health Corporation (4), Devoted Health, Inc. (3), Health Care Service Corporation (2) file the most open segments. Highmark Health is the parent of the state's Blue Cross Blue Shield plan. Special needs plans number 14: 6 for people with Delaware Medicaid, 7 for chronic conditions and 1 for nursing-home residents. Compare Maryland, Pennsylvania and New Jersey.
