Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read

Residents of the District of Columbia who have Parts A and B can choose among 11 Medicare Advantage plans open to the public for 2027, against 16 in 2026. Only 2 of the 8 plans that include drug coverage charge no monthly premium, a much smaller share than in the surrounding suburbs. In-network out-of-pocket caps run from $5,700 to $7,900, with a median of $7,150, above the national $6,550. Quality scores are high: 8 of the 11 plans hold four stars or more for 2027. Kaiser Foundation Health Plan, Inc. files the most open plans, followed by CVS Health Corporation and CareFirst, Inc.. 7 plan IDs offered in 2026 were not renewed and 2 are new. The District also has 4 special needs plans and 9 stand-alone Part D plans, the cheapest at $5.30 a month, listed below.

A city-sized market with few $0 plans

Because DC counts as one county, every resident sees the same 11 open plans, whether they live in Anacostia or Chevy Chase. Kaiser Foundation Health Plan, Inc. (4), CVS Health Corporation (3), CareFirst, Inc. (2), Humana Inc. (1), UnitedHealth Group, Inc. (1) file them. Only 2 carry no monthly premium with drug coverage, so many DC members pay a plan premium on top of Part B. The lowest cap, $5,700, comes with Kaiser Permanente Medicare Advantage High DC (HMO-POS) at $115 a month; the top premium among open plans is $115.

Kaiser's position in the District

Kaiser Permanente runs its own medical centers in the region and files 4 of the open plans. Its HMO-POS plans let members use some care outside Kaiser for a higher share of the bill. Kaiser Permanente Medicare Advantage Liberty (HMO), rated 4.5 stars, costs $0 with a $5,900 cap. Members who rely on a university hospital or a specific downtown practice should check each directory, because DC networks differ more from plan to plan than the short list suggests.
