Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read
Maryland has 30 Medicare Advantage plans open to anyone with Parts A and B for 2027, after 38 in 2026, one of the sharper drops in the country in proportion. Only 7 of the 23 drug-covering plans have a $0 premium, and caps are high: no open plan goes below $5,700, and the median is $7,500. Montgomery County leads with 20 open plans, while Garrett, St. Mary's and Allegany on the Eastern Shore list 4 each. The insurers are partly local: Kaiser Foundation Health Plan, Inc., Humana Inc., LifeBridge Health, Inc., CareFirst, Inc.. Quality is solid, with 17 plans at four stars or more and a best rating of 4.5. Maryland's hospitals work under a statewide all-payer rate system, which shapes how plans contract. For people on Original Medicare, 9 stand-alone drug plans are sold, the cheapest at $5.30 a month.
Health systems that sell their own plans
Kaiser Foundation Health Plan, Inc. (7), Humana Inc. (6), LifeBridge Health, Inc. (5), CareFirst, Inc. (5), Johns Hopkins Healthcare LLC (5), Health Care Service Corporation (2) file the most open plans in Maryland. Kaiser Permanente runs its own clinics around Washington and Baltimore, and its HMOs only work inside that system: the best-rated Maryland plan, Kaiser Permanente Medicare Advantage Value 1 MD (HMO), is one of them, at $0 with a $8,200 cap. Johns Hopkins files plans built around its hospitals. Before choosing one of these, ask a simple question: is your current doctor part of the system?
The price of a lower cap
The lowest cap in the state, $5,700, belongs to Kaiser Permanente Medicare Advantage High MD (HMO-POS), which charges $114 a month. In Maryland, a lower worst case usually comes with a premium, which makes the whole-year view more useful than the monthly line.

Suburbs and the Eastern Shore
Montgomery (20 open plans, 5 at $0), Anne Arundel (18 open plans, 2 at $0), Howard (18 open plans, 3 at $0), Baltimore (17 open plans, 1 at $0) have the widest choice. Baltimore County lists 17, but only 1 with drugs at $0. Across the Bay Bridge, Garrett, St. Mary's, Allegany counties have about half as many. Residents near Delaware or Virginia can only join plans filed for their own county, even if their doctors are across the line: compare with Delaware, Virginia and DC.
Maryland also lists 17 special needs plans, 13 of them for people who have Medicaid as well. Stand-alone drug plans went from 10 to 9.
