Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read

Louisiana has 70 Medicare Advantage plans open to anyone with Parts A and B for 2027, spread over 64 parishes, and 47 of the 64 drug-covering ones charge no monthly premium. Next to that open list sit 54 special needs plans, 34 of them D-SNPs for people with both Medicare and Medicaid, a large share for a state of this size. The lowest in-network cap is $2,875, for Humana LCMC Advantage H1951-051 (HMO), a plan co-branded with a New Orleans hospital system, at $0; the median is $6,300. Orleans Parish lists the most open plans, 34, with East Baton Rouge at 33, while Concordia Parish has 15. 37 plans earn four or more stars for 2027. Louisiana's stand-alone drug plans are cheap by national standards: the median of the 8 plans is $26.70 a month.

Plans built for dual eligibility

About one Louisiana plan in 4 on the full list is a D-SNP, a special needs plan for people who receive both Medicare and Louisiana Medicaid. East Baton Rouge Parish alone has 24. These plans coordinate the two programs, and for people whose Medicaid pays Medicare cost sharing they can bring copays close to zero; some also add transportation or an allowance for over-the-counter items. If you have Medicaid, you have more chances to change plans during the year than other beneficiaries. Low-income members also get Extra Help with drugs; the tool shows what that leaves at the pharmacy in 2027.

Chronic-condition and nursing-home plans

The other special needs plans are 17 C-SNPs, for conditions such as diabetes, heart failure or COPD, and 3 I-SNPs for nursing-home residents. See how special needs plans work.

New Orleans, Baton Rouge and rural parishes

Orleans (34), East Baton Rouge (33), Jefferson (33), Ascension (32), St. Charles (32) lead the parish table. North and central Louisiana have shorter lists: Concordia has 15, Caldwell 16 and Claiborne 16. Plans tied to a hospital system, such as the LCMC-branded Humana plans in the New Orleans area, are only as good as your access to that system, so look at the hospital list before the premium.

The insurers

Humana Inc. (28), Louisiana Health Service & Indemnity Company (11), CVS Health Corporation (9), Devoted Health, Inc. (9), UnitedHealth Group, Inc. (7), Centene Corporation (6) file the most open segments. Louisiana Health Service & Indemnity Company is Blue Cross and Blue Shield of Louisiana. Humana USAA Honor Giveback (HMO) reaches 4.5 stars with a $0 premium and a $4,900 cap. HMOs number 40 and PPOs 30. The open list went from 71 plans to 70, and premiums reach $249 at the top.

For people who keep Original Medicare, the cheapest drug plan is Wellcare Value Script (PDP) at $5.30, and 3 plans are free with full Extra Help. Compare Mississippi, Texas and Arkansas.
