Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read
Kentucky has 80 Medicare Advantage plans open to anyone with Parts A and B for 2027, and 47 of the 72 plans with drug coverage charge no monthly premium. Northern Kentucky leads: Boone, Campbell and Kenton counties each list 43 open plans, ahead of Jefferson County, Louisville, with 37 and Fayette, Lexington, with 38. The thinnest county is Hardin, around Elizabethtown and Fort Knox, with 19. The lowest in-network cap, $3,900, belongs to AARP Medicare Advantage from UHC OH-19 (HMO-POS), a plan numbered for Ohio that also serves Kentucky, at $35 a month; the median cap is $6,925. Quality is a strong point: 52 of the 80 plans hold four stars or more for 2027. Humana, based in Louisville, files 31 of them. Kentucky also has 43 special needs plans and 8 stand-alone drug plans.
Cincinnati's suburbs set the pace
Boone, Campbell and Kenton counties share the Cincinnati market, so insurers that build networks around Ohio hospitals extend them south of the river. Residents there get 43 open plans and a lowest cap of $3,900. A plan whose name carries an Ohio code can be filed for Kentucky counties too; what matters is whether your county appears in its service area and whether your hospital, on either side of the river, is in its network.
Louisville, Lexington and the rest
Jefferson County lists 37 plans with 18 at $0, Fayette 38 with 19. Pike County, deep in the eastern coalfields, still has 32; the shortest lists are scattered across the state rather than grouped in one region. Hardin County, with Fort Knox, has 19, and Laurel 20. Every Kentucky county keeps several $0 plans with drug coverage.
A home-state insurer and high ratings
Humana Inc. (31), Elevance Health, Inc. (14), UnitedHealth Group, Inc. (13), CVS Health Corporation (9), Devoted Health, Inc. (5), Centene Corporation (3) file the most open segments; Elevance Health is the parent of Anthem. Humana Value Choice (PPO) carries 4.5 stars at $0 with a $7,150 cap. With 52 plans at four stars or more, a Kentucky shopper can usually narrow the list by rating first and then compare caps and drug lists. HMOs and PPOs split almost evenly, 39 and 40, plus 1 private fee-for-service plan.
Joining Part D late
People who keep Original Medicare choose among 8 stand-alone drug plans, from $5.30 a month (Wellcare Value Script (PDP)), with a median of $74.35. If you go 63 days or more without creditable drug coverage after your first enrollment window, a penalty is added to every future Part D premium. The tool below estimates it for 2027.
