Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read

The figure that matters most in New Jersey for 2027 is not the premium but the cap: the median in-network out-of-pocket maximum among the state's 56 open Medicare Advantage plans is $9,750, against $6,550 nationally and a federal ceiling of $9,850. Choice itself is broad and even. Every one of the 21 counties has at least 28 open plans, and Camden County leads with 41. 25 drug-covering plans charge a $0 premium, and the lowest cap filed is $4,150, so low-cap plans exist; they are simply the minority. 38 plans hold four stars or better for 2027, and the top rating is 5. 15 special needs plans and 11 stand-alone drug plans, with a median premium of $120.50, are listed below.

Why so many New Jersey plans sit near $9,850

Each year CMS sets an upper limit on what a plan may charge you for covered in-network care, and an insurer may choose any cap at or below it. In New Jersey most chose to stay near the top. Half of the open plans have a cap of $9,750 or more. That design keeps premiums and copays low in a normal year, and shifts the risk onto the member in a bad one: a cancer treatment, a joint replacement with rehab, a long cardiac stay. The calculator below starts from the state's median cap.

Against Original Medicare alone, which has no yearly limit at all, any cap is protection. Against a Medigap policy, which covers nearly all of the 20% coinsurance, a $9,750 cap is a real exposure, and that comparison is the one many New Jersey retirees face.

A star plan with a high ceiling

The highest-rated open plan, Clover Health Choice Giveback (PPO), holds 5 stars and a $0 premium, with a cap of $9,850. Ratings measure quality and member experience, not your worst-case bill, so the two columns in the county table deserve to be read together. For a lower ceiling, HumanaChoice Giveback H5216-116 (PPO) has the state's minimum at $4,150.

From Camden to Hunterdon

South Jersey has the deepest menus: Camden 41, Burlington 39, Gloucester 39. Camden County alone has 17 plans at $0. The thinner end is still generous: Hunterdon 28, Ocean 29, Warren 29. CVS Health Corporation (16), Humana Inc. (11), UnitedHealth Group, Inc. (9), Clover Health Holdings, Inc. (8), Devoted Health, Inc. (3) file the most plans. The total went from 72 in 2026 to 56, 16 fewer plans; county pages list the plan IDs that ended.

Questions people ask

What is the average out-of-pocket maximum for Medicare Advantage in New Jersey in 2027?

The median cap among New Jersey's 56 open plans is $9,750 for in-network care, much higher than the national median of $6,550. The range runs from $4,150 to $9,850, per the CMS landscape file of September 2026. Drugs are capped separately at $2,400 for the year.

Which New Jersey county has the most Medicare Advantage plans for 2027?

Camden County leads with 41 open plans, followed by Burlington and Gloucester with 39 and 39. The smallest menu, in Hunterdon County, still holds 28. In New Jersey the gap between counties is narrow, so network and cap usually separate plans more than availability does.

Is a $0 premium plan in New Jersey worth it with a high cap?

That depends on your health spending, which only you can weigh. 25 New Jersey plans with drug coverage have a $0 premium for 2027. In a year with little care, the premium saving dominates; in a year with a hospital stay, the cap sets the bill. The whole-year cost calculator shows both cases for any plan.
