Virginia has 106 Medicare Advantage plans open to anyone with Parts A and B for 2027, a few more than the 103 of 2026, spread over 133 counties and independent cities, which CMS treats as separate areas. 66 of the 94 drug-covering plans charge a $0 premium. The lowest cap is $2,250, for Humana Gold Plus H5619-090 (HMO) at $106 a month, and the median $7,150, above the national $6,550. Central Virginia and Hampton Roads have the longest lists: Richmond City, Chesapeake City, Henrico with 42 or more. Northern Virginia has fewer than you might expect, 25 in Fairfax County, and the Shenandoah Valley and the Eastern Shore close the list, with 12 in Harrisonburg City. Humana Inc., UnitedHealth Group, Inc., CVS Health Corporation file the most plans. 69 are rated four stars or more. 9 stand-alone drug plans are sold, from $5.30 a month.
Counties and independent cities
Virginia is one of the few states where cities are not part of a county, and each one is a separate area for Medicare plans. Richmond city lists 43 open plans, 25 at $0 with drugs; the surrounding Henrico County has a similar list, but some smaller cities differ from the county around them. When you search, pick the city if you live within its limits: a plan filed only for the county does not cover you there. Your mailing address can name a city you do not live in, so check the locality that collects your property tax or appears on your voter card.

Northern Virginia and the military
Fairfax County lists 25 open plans, only 9 of them free with drugs, with a lowest cap of $5,900. Many retirees there have federal or military coverage: TRICARE For Life works with Original Medicare, and federal retirees can keep FEHB or PSHB coverage, which changes the math of an Advantage plan. Several plans on the list are giveback PPOs aimed at veterans, such as the state's best-rated, AARP Medicare Advantage from UHC VA-0005 (PPO), at $0 with a $7,900 cap.
Hampton Roads and Southwest Virginia
Richmond City (43), Chesapeake City (42), Henrico (42), Petersburg City (42) lead the state. Harrisonburg City (12), Shenandoah (14), Accomack (19) have the shortest lists. Virginia counts 42 special needs plans, 18 for people with Medicaid, and 3 private fee-for-service plans. In the southwest, where Bristol and Norton sit on state lines, check that a plan's network reaches the Tennessee or Kentucky hospital you use. Neighbors: Maryland, DC, North Carolina, West Virginia.
