Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read

Massachusetts has 76 Medicare Advantage plans open to anyone with Parts A and B for 2027, against 81 in 2026, and 60 of them carry a 2027 rating of four stars or more, one of the highest shares in the country. The market belongs to regional health plans rather than national brands: Point32Health, Inc. (27), Fallon Community Health Plan, Inc. (11), CVS Health Corporation (9), Baystate Health, Inc. (8). Free plans are the exception, with 20 of the 61 drug-covering plans at a $0 premium, but caps are lower than in most of the Northeast: from $3,400, with a median of $5,750. HMOs account for 56 of the open plans. Hampden County has the longest list (37), Barnstable, on Cape Cod, the shortest (16). For people on Original Medicare, 10 stand-alone drug plans are sold, with a median premium of $114.75, among the highest medians in the country.

A market of regional nonprofits

Point32Health, the parent of Tufts Health Plan and Harvard Pilgrim, files 27 open plan segments, Fallon Health 11, and Baystate Health, through its Health New England plans, focuses on the western counties. These organizations contract closely with local hospital systems, which explains both the high ratings and the narrow networks of some HMOs: a plan built around the Worcester or Springfield hospitals may not include Boston's teaching hospitals, and the reverse.

Lower caps, but at a price

The lowest in-network cap, $3,400, belongs to Fallon Medicare Plus Blue (HMO), at $117 a month. The best-rated widely available plan, Medicare HMO Blue SaverRx (HMO-POS), costs $0 but caps spending at $9,200. The two ends of the trade are both on the list; the whole-year calculator says which one suits your expected care.

From Hampden to Cape Cod

Hampden (37), Bristol (36), Essex (35), Norfolk (31) have the most open plans; Middlesex County lists 30, 8 of them free with drugs. Nantucket (3), Berkshire (7), Dukes (8) have the fewest. On Cape Cod, where many retirees spend part of the year elsewhere, the shorter list and the HMO networks make a PPO or Original Medicare with a supplement worth comparing.

Drug coverage is expensive here if you buy it alone: the median stand-alone plan costs $114.75 a month, against $13.30 for the cheapest. Massachusetts also lists 17 plans for people with MassHealth as well as Medicare. People who split the year with Florida or another state should weigh how far each network reaches. Neighbors: New Hampshire, Rhode Island, Connecticut.
