Caps run from $3,400, for Fallon Medicare Plus Blue (HMO) at $185 a month, to $9,850, with a median of $6,725. Quality is high: 23 plans carry four stars or more for 2027. HMOs dominate with 21 plans against 9 PPOs. Point32Health, Inc., Blue Cross and Blue Shield of Massachusetts, Inc., CVS Health Corporation, Fallon Community Health Plan, Inc. file the most. The county also lists 20 special needs plans, 17 of them for people with MassHealth. The list above shows every open plan with its premium, drug deductible and cap.

The two ends of the list are far apart. Fallon Medicare Plus Blue (HMO) limits in-network spending to $3,400 but costs $185 a month. The best-rated plan, Medicare HMO Blue SaverRx (HMO-POS), costs $0 but caps spending at $9,200. Over a calm year the second wins; in a year with surgery the first does.

Boston hospitals and network lines
Many Middlesex residents use hospitals in Boston, in Suffolk County, which lists 30 open plans of its own, and Essex County to the north lists 35. You join plans filed for the county where you live, but the network decides which hospitals you can use: an HMO built around a community hospital system may not include the Boston teaching hospitals.
