Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read

For 2027, Arkansas residents with Parts A and B can pick from 54 Medicare Advantage plans open to the public, 37 of which combine drug coverage with a $0 monthly premium. The lowest in-network cap is $4,000, set by Essence Advantage (HMO), and the median is $6,750, compared with $6,550 for the country. PPOs outnumber HMOs, 30 to 21, and 3 private fee-for-service plans remain on the list. Northwest Arkansas and the Little Rock area have the deepest menus: Benton County shows 44 open plans and Pulaski 43. The Delta and the southeast are thinner, with Drew County at 13. 23 plans hold four or more stars for 2027. People who prefer Original Medicare can choose among 11 stand-alone drug plans, priced from $5.30 a month.

Private fee-for-service: a rare plan type that survives here

Arkansas lists 3 PFFS plans for 2027. A private fee-for-service plan sets what it pays doctors and hospitals, and you can see any provider who accepts those payment terms and agrees to treat you; the provider can decide at each visit (Medicare.gov, Private Fee-for-Service plans). That flexibility helps in counties where no HMO has built a network, but it carries a risk: a clinic that took the plan last month can decline it today. Ask your doctors before choosing one, and check whether the plan includes Part D, because if it does not you need a separate drug plan.

Why no Arkansas cap goes below $4,000

The floor of $4,000 comes from Essence Healthcare, an HMO at $0. Arkansas has fewer of the tightly managed HMOs that file very low ceilings in larger markets. The median, $6,750, means half the open plans could leave you paying more than that in a year of heavy treatment, before drug costs.

Bentonville and Little Rock against the Delta

Benton County has 44 open plans and 31 at $0 with drugs. Pulaski follows with 43. In Drew, Desha and Ashley counties, the list drops to 13 to 15 plans, mostly national PPOs. The county table shows every figure.

The companies behind the plans

Humana Inc. (16), CVS Health Corporation (13), Health Care Service Corporation (7), UnitedHealth Group, Inc. (6), USAble Mutual Insurance Company (4), Devoted Health, Inc. (3) file the most open segments. USAble Mutual Insurance Company is the Arkansas Blue Cross parent. The top rating for 2027 is 4.5 stars, held by Aetna Medicare Eagle Giveback (PPO) with a $7,900 cap; a plan with "giveback" in its name returns part of the Part B premium to members.

Arkansas counts 20 D-SNPs for people with both Medicare and Medicaid and 9 C-SNPs for chronic conditions. The open list moved from 56 plans in 2026 to 54, 2 fewer plans. Among drug plans, 1 charges no deductible.
