Utah has 33 Medicare Advantage plans open to anyone with Parts A and B for 2027, down from 38 in 2026. 18 of the 27 drug-covering plans charge no premium, the lowest cap is $4,700, for Humana Total Complete (HMO), and the median is $6,700. Quality is high: 29 of the 33 plans carry four stars or more for 2027. Choice follows the Wasatch Front, where Davis, Salt Lake, Utah counties list 28 or more open plans each; Carbon, Piute, Sanpete counties list 6, and Daggett and Wayne counties have no open plan at all. PPOs outnumber HMOs, 23 to 10. Humana Inc., UnitedHealth Group, Inc., CVS Health Corporation file the most plans, and Intermountain Health, the state's largest hospital system, files its own. 8 stand-alone drug plans are sold, from $5.30 a month.
Intermountain and the hospital question
In Utah, choosing an Advantage plan is largely choosing which hospital system you will use. Intermountain Health runs many of the hospitals along the Wasatch Front and sells plans through its own insurer, SelectHealth, while University of Utah Health and other systems contract with the national carriers. A plan's network decides whether your surgeon at one system or the other is covered. The Humana Inc. (11), UnitedHealth Group, Inc. (9), CVS Health Corporation (5), Devoted Health, Inc. (3), Intermountain Health Care, Inc. (3), Cambia Health Solutions, Inc. (2) plans of 2027 do not all include both.

Salt Lake County, and the long drive south
Salt Lake County lists 28 open plans, 15 of them at $0 with drugs, and a lowest cap of $4,700. In the southern and eastern counties, where St. George, Moab or Vernal residents may travel hours for a specialist, the list is short and made mostly of PPOs, which pay out of network at a higher share. In Daggett and Wayne counties, Original Medicare with a drug plan and Medigap is the only route; a move into one of them ends an Advantage plan and opens a special enrollment period.
Ratings
The best 2027 rating, 4.5 stars, goes to plans such as Humana Value Choice (PPO), a $0 plan with a $5,000 cap: a high rating does not always come with a low cap, so read both columns before you decide. Utah also lists 18 special needs plans, 11 for people with Medicaid. Neighbors: Idaho, Nevada, Colorado, Arizona.
