Compiled by Radif Partners from the CMS landscape file of September 2026 · How the data is read
Wisconsin has 89 Medicare plans open to anyone with Parts A and B for 2027, against 94 in 2026, filed by 12 organizations, more than in most states of its size. The median in-network cap is $5,650, well under the national $6,550, and the lowest is $2,750, for Dean Advantage Complete (HMO) at $276 a month. Free plans are less common than in the South: 28 of the 59 drug-covering plans. The list includes 14 cost plans, which let Original Medicare pay for care outside their network. The insurers are partly local: Humana Inc., Medica Holding Company, University of Wisconsin Hospitals and Clincs Autho, UnitedHealth Group, Inc.. The northeast has the longest lists, Shawano (37) and Sheboygan (35), while Richland, Bayfield, Vernon in the southwest list 10. 63 plans are rated four stars or more. Stand-alone drug plans are cheap here, with a median premium of $10.85.
Health systems that sell plans
Dean Advantage, linked to SSM Health, Quartz, owned in part by UW Health and Gundersen, and Medica file many Wisconsin plans next to Humana and UnitedHealthcare. Their plans are built around specific hospitals: Madison's, La Crosse's or Green Bay's. Dane County lists 31 open plans, with the lowest cap in the state, $2,750, but only 5 at $0 with drugs; Milwaukee County lists 33, 17 of them free. The difference reflects which systems contract in each county.
What an out-of-pocket cap is worth in a bad year
The cap saves
$8,372
| Original Medicare alone (20%, 1 admission) | $14,022 |
| With the plan’s cap | $5,650 |
Cost plans next to Advantage plans
The 14 cost plans on the list keep a network for routine care but let you see any provider who accepts Medicare, with Original Medicare paying as usual for out-of-network care. They usually carry a premium and often no drug coverage, so members add a stand-alone Part D plan. In Wisconsin, with a median stand-alone premium of $10.85, that addition costs less than in many states.
Ratings and the trade between premium and cap
The best 2027 rating, 4.5 stars, goes to plans such as Aetna Medicare Eagle (PPO), at $0 with a $5,200 cap. The lowest caps in Wisconsin come with premiums, as with Dean Advantage Complete (HMO) at $276: someone expecting a surgery may save money paying that premium, someone in good health usually will not.
From Green Bay to the Driftless
Shawano (37), Sheboygan (35), Waupaca (34), Brown (33) lead; Richland (10), Bayfield (11), Vernon (11) close the list, where La Crosse, Dubuque or Madison hospitals may be the nearest. Wisconsin counts 28 special needs plans, 22 for people with Medicaid. Neighbors: Minnesota, Michigan, Illinois, Iowa.
