Compiled by Radif Partners from the CMS landscape file of September 2026 · all California counties and drug plans

San Bernardino County offers 49 Medicare Advantage plans open to anyone with Parts A and B for 2027 and, alongside them, 46 special needs plans: 38 for chronic conditions such as diabetes, heart failure or lung disease, 6 for people with Medicare and Medi-Cal, and 2 for nursing-home residents. Among the open plans, 40 include drug coverage at a $0 premium, the lowest in-network cap is $295 (Imperial Dynamic Plan (HMO)), the median cap is $1,800, and no plan charges more than $49 a month. 21 plans have four stars or better in the 2027 ratings, led by Kaiser Permanente Sr Advantage Inland Empire Value (HMO) at 4.5 stars. The 2026 file listed 58 open plans; 13 IDs were not filed again and 4 were added. Humana, with 8 plans, files the most.

Chronic-condition plans: who qualifies

A chronic-condition SNP (C-SNP) accepts only members with a diagnosis the plan names, confirmed by a doctor shortly after enrollment. In exchange, the benefits are shaped around that illness: care coordinators, drug lists centered on its treatments, sometimes lower copays for the visits it requires. With 38 such plans in the county, a resident with diabetes or heart disease has more choice inside this group than many counties offer in total. A C-SNP is not a better plan by default; compare its cap and network with the open plans that serve the same doctors.

The cost of one hospital stay without a plan

Hospital stays are where chronic illness costs the most. Under Original Medicare in 2026, the Part A deductible is $1,736 per benefit period, and very long stays add a daily charge of $434. An Advantage plan replaces that with daily copays, limited by the yearly cap.

A county the size of a state

From Ontario and Rancho Cucamonga to Victorville, Barstow and Needles, San Bernardino spreads over a huge territory, and networks thin out in the High Desert and the mountain communities. Kaiser Permanente sells the same Inland Empire Value plan here and in Riverside County, which lists 47 open plans. Los Angeles County, to the west, has 70; Kern, to the north, 21. Central Health plans and Molina Medicare Choice Care are among this year's exits, listed at the bottom of the page.
